ABSTRACT

In the course of the study, histological preparations of 35 patients aged 16­25 years with incisor crowding who underwent orthodontic treatment with the fixed appliances according to the standard technique, had been analyzed. Three groups of patients had been distinguished: the first one was the reference group, the second one was the group of patients with the fiberotomy conducted before treatment with fixed orthodontic appliances, and the third one was the group of patients with the fiberotomy performed after the removal of fixed orthodontic appliances according to a standard technique used in the Department of Maxillofacial Surgery and Dentistry of Samara State Medical University (SamSMU)

INTRODUÇÃO: o problema transversal em Ortodontia pode ser de origem dentária, esquelética ou uma combinação de ambas e pode ser tratado de várias maneiras de acordo com o tipo de tecido envolvido e o arco dentário acometido. Em se tratando de um problema esquelético e maxilar, o ortodontista poderá lançar mão da disjunção ortopédica, dependendo da idade óssea, obtendo uma correção da atresia, um alinhamento dentário espontâneo resultante do aumento no perímetro do arco e um sorriso mais amplo, beneficiando esteticamente o paciente. Porém, este tratamento de disjunção mandibular se torna inviável, já que este osso tem sua sutura fusionada precocemente. A expansão neste caso é alveolar e sua estabilidade é questionada. OBJETIVO: relatar um caso clínico, apresentando uma nova alternativa de tratamento para o apinhamento inferior: a Distração Osteogênica da Sínfise Mandibular (DOSM). Neste procedimento, é feita uma osteotomia sagital na sínfise, criando-se uma sutura artificial para que uma separação óssea seja conseguida, utilizando uma mecânica com aparelho disjuntor de Hyrax modificado. RESULTADOS: os resultados do tratamento apresentado mostram uma melhora na forma do arco, diminuição do corredor bucal, correção do apinhamento dentário e também uma estabilidade do tratamento, sem dano aos tecidos adjacentes. Neste caso clínico, a DOSM se mostrou uma excelente opção de tratamento.


INTRODUCTION: The transversal problem can have dental or bone origin or a combination of both and can be treated in many ways, in accordance with the type of tissue involved and the dental arch affected. When dealing with a bone and maxilla problem, the orthodontist could use the orthopaedic separation of the palatal suture, depending on the bone age of the patient, thus obtaining a correction of the atresia, a spontaneous dental alignment resulting from the increase of the arch's perimeter and a more ample smile which will esthetically benefit the patient. But this mandible separation treatment becomes unavailable since this bone has its suture fused at an early age. The expansion in this case is alveolar and its stability is questioned. OBJECTIVES: to relate a clinical case presenting a new alternative for the treatment of inferior crowding: the Osteogenetic Distraction of the Mandibular Symphysis (ODMS). In this procedure, a sagittal osteotomy of the mandibular symphysis is performed, creating an artificial suture so that a bone separation is obtained using the mechanics of a modified "Hyrax" separator. RESULTS: the patient showed an improvement in the arch, reduction of the buccal corridor, correction of the dental crowding and also a stability of the treatment without damage to the surrounding tissues. In this case, the ODMS showed to be an excellent treatment option.

ABSTRACT

Mandibular incisor crowding is one of the most common features of malocclusion and is interesting characteristic in view of relapse and stability after orthodontic treatment. There are many potential factors in the etiology of lower anterior crowding. The tooth size variation is one of them, but biologic significance for the faciolingual width of the teeth has been overlooked. Peck and Peck reported that persons with ideal mandibular incisor alignment were shown to have incisor with smaller mesiodistal and larger faciolingual dimensions than persons with incisor crowding. On the basis of these findings they suggested MD/FL index as a clinical guideline for the assessment for lower incisor crowding. The present study was undertaken to examine the relationship between mandibular incisor crowding and mandibular incisor dimension, and determine their correlation with arch length discrepancy. 154 dental casts of people from 11 to 17 years of age were made, and were divided into normal group with irregularity index less than of 1, and crowding group with irregularity index greater than 1. The casts were measured and analyzed statistically. The results were as follows. 1. The mean mesiodistal width for mandibular incisor was larger in crowding group, and has significant difference in central incisor measurement. There are no significant differences in the faciolingul width and MD/FL index. 2. Irregularity index has significant correlation coefficients with mesiodistal width and MD/FL index for mandibular incisor in crowding group, but no correlation with faciolingual width. It also has correlation with maxillary and mandibular arch length discrepancy, total tooth material, mandibular intercanine width, and mandibular inter first premolar width. 3. Upper and lower arch length discrepancy have significant correlation with mesiodistal width of mandibular incisor and overbite, but have no correlation with faciolingual width. Lower arch length discrepancy has significant correlation with MD/FL index for mandibular incisor and upper arch length discrepancy has correlation with MD/FL index for mandibular lateral incisor. 4. Significant differences were observed between normal and crowding group for the mandibular arch length discrepancy and overbite.
